An inquest has been launched to examine the 2013 death of a man at a psychiatric facility in Penetanguishene.

Dr. William Lucas, regional supervising coroner for central region announced on Wednesday that an inquest will be held to examine the death of Richard Swain.

According to Lucas, the 36-year-old died on May 15, 2013, while restrained at the facility.

The inquest is mandatory under the Coroners Act and will examine the events surrounding Swain’s death.

A jury will also make recommendation in an effort to prevent similar deaths in the future.
